This one-time investment of $700 includes a 90 minute in-person appointment with Dr. Claxton where your health timeline is reviewed and interpreted through Functional Medicine lens. Advanced biomarker (lab) testing is interpreted and recommendations with support for taking steps towards improving health and longevity, and two months of online portal support with these goals in mind.

This is designed as a one-time assessment for optimal wellness.

Membership Plan

For those who have completed the Foundational Assessment and choose to continue to benefit from our services and education at Flagstaff Functional Medicine and Functional Medicine Sedona.
During the Foundational Assessment, Dr. Claxton will discuss whether we are a good fit for each other and whether you would benefit from our membership. Completion of the Foundational Assessment DOES NOT guarantee a spot in our membership plan.
